If your agency is planning on adding a branch to your current state license and requesting certification of the Branch for Medicare purposes, please read this important message from the Arizona Department of Health Services.
There are 2 processes that both must be followed in sequence when adding a Branch to a current state license and requesting certification of the Branch for Medicare purposes.
Process 1 - State Licensing process to add a branch to an existing licensed Home Health Agency:
The Facility must request in writing to amend their current license by adding a branch office to their current Home Health Agency license (Parent).
The Department will review this request for a branch and may choose to conduct an onsite survey to ensure there is appropriate personnel and resources available to the branch site and that the care and services are equal to the Parent site.
The Department will make a determination and the facility will be notified of the determination.
When the determination is made to add the branch to the Parent license, the license will be amended and at that time the branch may begin to operate. (Branch can not operate without approval).
The Parent Agency will receive an amended license that will include the address of the branch and the branch will be issued a license indicating that they are under the license of the Parent and this is posted in the branch.
This is a very important first step because CMS will not certify a branch under the Medicare Program unless the Department has taken action on the state licensing side.
Process 2: CMS Process for certifying a Branch
The Facility will notify ADHS of the request to add a Branch Office to their certified Home Health Agency.
The Facility will submit the 855 to the FI for approval.
Once the FI returns their recommendation, the SA (ADHS) will make recommendation to CMS regarding certification of the Branch. (CMS will be considering the SA action related to licensing as part of the approval process for certification)
CMS will review the documentation and recommendation from the SA and make a determination as to the certification of the branch.
CMS will notify the SA and the Facility of the final decision for certification.
