Why You Should Attend

Investing in education has shown to have a significant impact on improved quality care for patients

and families. Plus spending quality time with healthcare community colleagues increases and

enhances your professional network.  The conference is specifically designed for professionals

caring for patients and families in a community or home-based care setting.

Leading from the Heart: Maintaining the Patient as True North

With the increased interest in the patient-centered care model as the key to achieving the IHI’s Triple Aim for Healthcare, it is essential that providers are not only inspired but equipped to provide care and guidance that is responsive to a patient’s wishes and consistent with their values.  Effectively navigating the complexities of chronic disease and end-of-life care while still meeting guidelines, requires a thoughtful, informed and engaged professional.

Join us for this 1-day conference for healthcare professionals in community-based care for end-of-life or advanced illness management.

Designed For:

Nurses, Physicians,Quality Improvement and Compliance Staff, Social Workers, Chaplains, Bereavement and other professionals working in a palliative care, advanced illness or end-of-life setting.

  • The pre-conference session is designed for any healthcare professional looking to improve their communication skills with patients with a serious illness or life-limiting condition.
  • The Clinical Review Course is designed for nurses who are preparing to take the HPNA CHPN Certification Test however, even those not planning on taking the exam will find the content of this course invaluable for application in their daily practice.
