Maricopa County Department of Public Health (MCDPH) issued a press release announcing an increasing number of monkeypox cases in the county. We currently have 10 cases with case investigation and contact tracing ongoing. MCDPH is instructing contacts of individuals who tested positive for monkeypox to monitor themselves for symptoms and providing post-exposure prophylaxis (JYNNEOS vaccine) to those with higher risk exposures.

It is important that our provider community consider monkeypox in their differential diagnosis for those who have a rash consistent with monkeypox and/or epidemiological risk factors for exposure to monkeypox. As most cases of monkeypox resolve without treatment and are not severe, referral for testing to local emergency departments/urgent care centers is not usually necessary.

Healthcare Alert: Methanol poisoning associated with alcohol-based hand sanitizer

The Arizona Department of Health Services was notified by the Food and Drug Administration (FDA) about several hand sanitizing products that have been inappropriately manufactured with methanol. Arizona Poison Control Centers have reported an increase in the number of methanol-related poisonings from hand sanitizers by ingestion and skin exposure.

Alcohol-based hand sanitizers have been marketed heavily during the COVID-19 pandemic as an alternative to washing hands with soap and water. FDA recently advised consumers not to use hand sanitizers manufactured by Eskbiochem SA de CV (made in Mexico), because even though the label may list ethanol they also can contain methanol. The FDA maintains a list of affected hand sanitizers that contain methanol.

The major toxic effects of methanol do not show until methanol is broken down in the body, which may be delayed for 12-24 hours after exposure. These effects can include: drowsiness, headache, vomiting, abdominal pain, visual disturbances, and blurred vision. Consumers who have been exposed to hand sanitizer containing methanol should seek immediate medical care.

If a patient presents with the aforementioned symptoms and has history of exposure to alcohol-based hand sanitizers, Arizona providers should:
● Provide immediate treatment, which is critical for potential reversal of toxic effects of methanol poisoning;
● Immediately call the Poison Control Center at (800) 222-1222.

Arizona providers are encouraged to educate patients on the appropriate use and storage of ABHS to avoid unintentional ingestion and exposures

The Arizona Department of Health Services and local health departments will continue to monitor developments related to this emerging issue. Please continue to check the CDC website for the newest, most reliable information. If you have any questions about reporting, please contact your local health department.

Arizona Health Alert Network - Contingency PPE and Testing Prioritization

In response to the COVID-19 pandemic, the Arizona Department of Health Services (ADHS) has convened the State Disaster Medical Advisory Committee (SDMAC). On March 24, 2020, guidance for personal protective equipment (PPE) preservation and testing prioritization was approved in accordance with the Arizona Crisis Standards of Care Plan.

Recommendation: Shift to Contingency PPE, Prepare for Crisis Measures

Standards for PPE use should be shifted from conventional to contingency measures; organizations sho

uld prepare for crisis measures. Recommendations for contingency use of different elements of PPE include extended use strategies. Further details can be found online under CDC’s Strategies for Optimizing the Supply of PPE.

Recommendation: Set Testing Prioritization

Given limited testing supplies, both reagents and swabs, and limited PPE, the SDMAC approved the following testing priorities. The goal of testing should be to limit introduction of COVID-19 in healthcare and congregate settings and vulnerable populations.

Public health and healthcare professionals should prioritize testing among three specific groups until sufficient and consistent PPE, testing supplies and capacity are widely available:

Community-based COVID-19 testing (drive-through, walk-up, etc.) should be focused on making tests available to the three priority groups above.

Healthy individuals and those with mild disease do not require testing. These individuals should practice social distancing and follow the recommendations of their local and state public health authorities.

More information on the SDMAC and the guidance reported here can be found on the ADHS website for SDMAC.

Coronavirus (2019-nCoV) and Action You Can Take:

The CDC is offering guidance to health professionals about the coronavirus and evaluating patients for home care or patients with symptoms who present at home. https://www.cdc.gov/coronavirus/2019-ncov/guidance-prevent-spread.html and https://www.cdc.gov/coronavirus/2019-ncov/guidance-home-care.html Attached also find a one page information sheet to use with staff, patients and other caregivers who may be increasingly concerned.

Please note, this is a good time to review handwashing practices with all staff, and also document your actions to support your emergency preparedness plan related to infectious disease, as the CDC has initiated outreach to local and state health departments-which are a resource to you and your staff.

The risk in the US remains low, however, there is now evidence of person-to-person transmission without the infected person having been in Wuhan China. The World Health Organization’s (WHO) designation of coronavirus as an emergency relates to this information and supports international action and resources to address the virus’ spread.

Immediately mask and place patients presenting with a febrile rash illness in airborne isolation if available, obtain vaccine and travel history.
