Do You Know a Heart Hero?

This Annual Program was developed to recognize individuals in palliative care and hospice who serve with their hearts and stand out among their peers.  They personify the qualities of compassion, inspiration, inclusion, responsiveness and empowerment and truly make a difference in the lives of the patients and loved ones they interact with everyday.


We invite our Members to nominate an individual from their organization using the online form located below.  

2020 Program:  Nominees will be invited to attend the Home Health & Hospice Awards Program being held in conjunction with the Southwest Conference.  Due to the pandemic, the program will be held virtually on November 19.  We will recognize all nominees and announce the Finalist for each category at the event.  


Finalists will also receive a $250 scholarship to be used towards a professional development program of their choice.

  • This recognition is intended for an individual that provides direct care to a patient and/or their family (minimum of 75% of time spent in a field level position).
  • Employed by or a volunteer for a Hospice or Palliative Care Provider (Supervisor's name and email will be required on the form).
  • If individual is licensed, the license is in good standing.

How to Submit a Nomination - Deadline Extended to October 2nd

Nominations will be accepted from AHPCO Members in good standing.  Each Member may submit one individual per award category for a maximum of 5 individuals total.  The nomination form must include the name and email of the Superivisor of the individual.  All nominations must be made electronically using the form below.  


The nomination form consists of the following:

  • Contact Info for Nominee (Name, Email and Phone - we will contact them to invite them as our Guest to the Awards Dinner)
  • Nominee's Role/Title and License type and number, if applicable.
  • Biographical Statement (150 words)
  • Photo (forward facing photo of the individual - preferred file types are JPG or PNG)
  • Statement: Describe why this individual deserves to be recognized as a heart hero in the category you are nomingting them for (compassionate, etc).
  • Statement: Describe the context in which you know the nominee including length of time known.

Helpful Tip: For the statement areas of the form - you may wish to compose the statements in a separate document and then "cut and paste" the content into the fields.
